Dr Derek Mahony

Dr Derek Mahony is a Specialist Orthodontist renowned for his contributions to early interceptive orthodontic treatment and airway health. With over 38 years of experience, Dr Mahony’s educational background reflects his dedication to comprehensive care in orthodontics. He earned a Bachelor of Dental Surgery from the University of Sydney and a Master of Science in Orthodontics from the University of London. His qualifications include a Diploma in Orthodontics from the Royal College of Surgeons in Edinburgh, memberships in Orthodontics and Dentofacial Orthopaedics from esteemed institutions, and a Fellowship in Orthodontics from the Royal College of Dentists in Canada. Dr Mahony has also completed Graduate Diplomas in Dental Sleep Medicine and Clear Aligner Therapy at the City of London Dental School, focusing on airway and craniofacial health.

 

Dr Mahony has built an international reputation through his clinical practice, research, and education. A dedicated researcher, he is an invited reviewer for multiple dental journals and has contributed to studies on facial development and nasal breathing. His research has highlighted the benefits of maxillary expansion in addressing systemic health issues, including bed-wetting, OSA and ADHD, showing the broader impacts of orthodontic intervention on patient health.

 

In addition to his clinical practice, Dr Mahony is a respected lecturer, sharing his expertise with orthodontic professionals, worldwide. Session Overview:

This lecture covers the Carriere Motion System philosophy, the appliance's design and biomechanics for Class III malocclusions, patient selection and appointment scheduling, the bonding/debanding procedure, managing patient compliance, the use of intermaxillary elastics, treatment outcomes and finishing protocols using fixed appliances or aligners, and case studies. The core concept is the "Sagittal First™" approach, addressing the sagittal discrepancy at the beginning of treatment by distalizing posterior mandibular segments to achieve a Class I platform.

I. Introduction to the Carriere Motion System

SAGITTAL FIRST™ Philosophy: Correcting the sagittal (forward-backward) discrepancy early in treatment for improved efficiency and patient compliance.

History and Evolution: Understanding the development of the appliance and its place in orthodontic treatment. 

Contraindications & Case Selection: Identifying suitable Class III patients and excluding those with significant Class III skeletal issues that would require surgery.

 

II. The Carriere Motion Class III (CM3) Appliance

Design and Features:

• Bilateral bars bonded to the mesial of the mandibular canines and lower first molars.

• Hooks for intermaxillary elastics.

• Incorporates a "ball and socket" feature on the upper first molar to guide its movement.

• Flat, molded pads to facilitate molar movement.

 Biomechanics:

• Distalisation of the entire mandibular posterior segment as a unit.

• Correction achieved primarily through dento-alveolar changes, with some mesialisation of lower molars.

 

III. Clinical Application

• Bonding and Debonding: Step-by-step procedures for appliance placement and removal.

• Patient Compliance: Strategies to ensure full-time elastic wear, which is crucial for treatment success.

• Anchorage: Utilizing an upper Essix retainer for anchorage to control the direction of forces. 

 

IV. Treatment Phases & Finishing

Phase I: CM3 Treatment: 

• Goal: Achieve a Class I platform by distalizing the mandibular molars.

• Duration: Typically 4-8 months.

• Monitoring: Regular progress checks (e.g., photos every 6 weeks).

Phase II: Finishing Treatment: 

• After achieving Class I, the CM3 is removed.

• The maxillary arch is bonded with fixed appliances or treated with aligners to complete the correction.
